Any CRNA who wishes to have their official transcripts sent to the NJ BON must:

                     email: wasneskir@lourdesnet.org

              or    mail: OLLMC NAP

                             1600 Haddon Ave.

                              Camden, NJ 08103

2)      enclose a $10.00 non-refundable processing fee for each request

        a.  Please make check out to “OLLMC NAP”

        b. we cannot accept debit or credit cards for this fee

3)      Transcripts will not be forwarded until the written request/processing fee is received

The OLLMC NAP is responsible for submitting official transcripts for all certificate prepared CRNAs. If the CRNA has earned a Master’s degree by either entry level or completion the CRNA must request the official transcripts be submitted to the BON by the degree granting institution. All anesthesia core courses will be reflected on Master’s degree transcripts.

All graduates of the OLLMC Program  have the requisite 39 pharmacology hours to satisfy the NJ BON requirements. You WILL NOT NEED TO SUBMIT the "Completion of Integrated Pharmacology" form with your APN Certification.
